Goal: The goal of the study was to prevent STDs in high-risk minority women through three culture-specific small group education and counseling sessions, delivered over time.

Impact: Reinfection rates of chlamydia and gonorrhea were significantly lower at each follow-up among participants in the small-group counseling sessions than in the control group. Integration of behavior-change theory with extensive qualitative data collected in target communities enabled the study to create culturally meaningful strategies to promote the recognition of risk and to stimulate motivation to effect personal change.

Goal: The objective of this article is to describe Project Restoration's approach to cross-sector collaboration with a shared population of high utilizing community members, describe the process and outcomes of the collaboration, provide strategies to mitigate challenges that arise during collaboration and offer insights to inform similar initiatives in communities nationwide.

Impact: Hospitals across the region are assessing ways to establish a centralized infrastructure to collaboratively address the complexities of individual cases, as well as a regional council to explore process improvements to enhance the lives of vulnerable populations.

Goal: The goal of Focus on Youth is to teach youth the skills and knowledge they need to protect themselves from HIV and other STDs.

Impact: The Focus on Youth intervention increased self-reported condom use and positively affected perceptions six months after the end of the program.

Impact: The Community Preventive Services Task Force (CPSTF) recommends tailored pharmacy-based adherence interventions for cardiovascular disease prevention. Evidence shows interventions delivered by pharmacists in community and health system pharmacies increased the proportion of patients who reported taking medications as prescribed. The CPSTF also finds these interventions are cost-effective for cardiovascular disease prevention.

Goal: The goal of the promising practice is to reduce binge-drinking behavior in college students using motivational interviewing and personalized feedback techniques.

Impact: At an eight-week follow-up, all four groups reduced their consumption, peak BAC, consequences, and dependence symptoms.
