Impact: The Community Preventive Services Task Force (CPSTF) recommends self-measured blood pressure monitoring interventions combined with additional support to improve blood pressure outcomes in patients with high blood pressure. Additional support may include patient counseling, education, or web-based support. Economic evidence indicates that self-measured blood pressure monitoring interventions are cost-effective when they are used with additional support or within team-based care.

Goal: Research shows that children benefit from kinship care in many ways. Kinship care can reduce the trauma that children may have previously endured and the trauma that accompanies parental separation by providing them with a sense of stability and belonging in an otherwise unsettling time. Children who have been placed with relatives may have experienced chronic neglect and physical, sexual, or emotional abuse. While these experiences place children at risk for behavioral and health problems, a positive relationship with a caregiver and a stable and supportive living environment can mitigate their impact.1 Grandparents, other relative caregivers, and “fictive kin” — close friends holding a family-like bond with a child — are in a unique position to fill this supportive role and promote resiliency.

The goal of Kinship Connections is to support kin families' social, emotional, and economic needs to increase placement stability within the child’s community. Specific program objectives are to improve family economic security, family relationship functioning, child well-being, and to increase kin caregiver social support.

Goal: The Memphis Enviromental Court was designed to serve as the chief vehicle for enforcing a wide range of laws relating to the health and quality of life of its residents, and to give new meaning to the term "expedient justice" by its ability to respond in a quick and consistent manner. In 1991 the Tennessee State Legislature created the Shelby County Environmental Court, and expanded the new court's powers to include the "injunctive authority" to mandate that defendants comply with the environmental codes.

Goal: To partner within communities and organizations to train professionals, community and parent leaders so the “Best Start” program can reach and support all families, but especially marginalized or struggling families and communities; to build the strong family/home, school and community environments that are necessary to support positive mental health, confidence, resilience and hope to achieve health equity, academic and economic opportunities for positive futures for all.

The Changing Children’s Worlds Foundation (CCWF) mission is for every child and adolescent to be supported in positive development by caregivers and professionals within loving, non-violent families and peaceful communities. Goal: CVAN is the Kansas conduit for tons of healthy produce from the Catholic Charities Mary Martha Organization--the steward of excess highly perishable high quality foods from a huge Walmart Distribution Center in Oklahoma. As food supplies allow, CVAN distributes to the 100 square mile school district and to share with a networked family of pantries and volunteer organizations in five Kansas counties and South Coffeyville, OK. As needed, CVAN has recruited distribution volunteers in covering communities within this economically depressed region. The network family was built upon the CVAN’s supply of fresh produce, but is now blessing this area with distribution of CARES foods during the COVID crisis. Its volunteers are food warriors--winning in the fight against rural hunger.
