Impact: Reduced 30-day readmission rate from 18.2 to 8.9 percent over the course of 2 years, resulting in estimated savings of more than $17 million through 1,804 avoided readmissions.

Impact: The Living Room is a comfortable, non-clinical space that offers an alternative to hospital emergency rooms. The program in LaGrange, Illinois hopes to decrease unnecessary hospitalizations.

Goal: The aim of the Montana Model is to provide health care to the 11 percent of the county citizens living below the federal poverty level and the many more living in near poverty, while simultaneously providing a medical clinic with diversity and complexity for family practice residents to obtain enriched training with the public health perspective.

Goal: Training for Adoption Competency (TAC) is a post-Master’s curriculum designed by C.A.S.E. with the assistance of a National Advisory Board of adoption experts. Through classroom and remote instruction as well as clinical case consultation, TAC students master 12 domains that are critical to adoption-competent mental health services.
