Promising Practices
The Promising Practices database informs professionals and community members about documented approaches to improving community health and quality of life.
The ultimate goal is to support the systematic adoption, implementation, and evaluation of successful programs, practices, and policy changes. The database provides carefully reviewed, documented, and ranked practices that range from good ideas to evidence-based practices.

The goal of this study is to determine how many Community Health Workers (CHW) would be needed to reduce emergency department (ED) visits and associated hospitalizations among their assigned patients to be cost-neutral from a payer's perspective.
This study adds significant knowledge to the existing literature on CHW programs, and particularly provides critical information to payers that can be used for making decisions on appropriate payment models

The goal of the program is to help families gain or increase parenting and family management skills that would facilitate successful child academic and social adjustment and, therefore, to promote social and academic competence and to lower risk for later antisocial behavior. In addition, the intervention concentrates on promoting initial academic success.
Children who received the intervention improved in overall reading ability at a more rapid rate than those who did not receive the intervention. In addition, SAFEChildren participants showed an improvement in concentration.

The Ambulatory Integration of the Medical and Social (AIMS) model aims to address social and environmental factors patients face that may prevent them from following their plan of care, thus impacting their health.
The AIMS model helps create better supported, less stressed, and better informed consumers and caregivers. There is also evidence to suggest that this model reduces ED usage and 30-day readmissions in participants.

The Community Preventive Services Task Force (CPSTF) recommends community mobilization combined with additional interventions such as stronger local laws directed at retailers, active enforcement of retailer sales laws, and retailer education with reinforcement on the basis of sufficient evidence of effectiveness in reducing youth tobacco use and access to tobacco products from commercial sources.

The updated CPSTF recommendation is based on findings from 27 studies in which vaccination programs in schools or child care centers:
-Provided vaccinations on site
-Were administered by a range of providers including school health personnel, health department staff, and other vaccination providers
-Were delivered in a variety of different school and organized child care settings
-Delivered one or more of a range of vaccines recommended for children and adolescents, and
-Included additional components such as education, reduced client out-of-pocket costs, and enhanced access to vaccination services
School- and organized child care center-located vaccination programs may be most useful in improving immunization rates among children and adolescents for new vaccines, and vaccines with new, expanded recommendations (such as the annual immunization for seasonal influenza) where background rates are likely to be very low and improvements in coverage are needed.
