Care Coordination Strategy
Activity 1: Assess usage of resource connection tool to be used within DCHCC member organizations
This is complete.
Activity 2: Partner with Capital Area Food Bank (CAFB) to pilot resource connection tool Aunt Bertha within Children’s National followed by DCHCC members.
This is complete.
Activity 3: Identify pros and cons of using Aunt Bertha or identify any additional resource tools
Ongoing assessment of challenges however; license has been purchased.
Activity 4: Meet with internal leadership to discuss incorporating within system (receive approval), provide guide and training to DCHCC members to discuss with respective leadership
The clinical leads have been identified.
Activity 5: Develop screening tool to detail effectiveness
Activity 6: Provide Children’s National and DCHCC members with overview and training on resource connection tool (Aunt Bertha); schedule workshops
The toolkit is complete (first version).
Activity 7: Begin Pilot Phase of Aunt Bertha training within Goldberg clinics, emergency department, and Social Work departments
The pilot is complete.
Activity 8: Evaluate user experience and identify areas of enhancement
We received feedback around initial communications and will continue to incorporate
Activity 9: Fully implement across DCHCC organizations and on DCHealth Matters
Clinical leads have been identified and kickoff call is being scheduled.
Activity 10: Identify areas of interest as well as opportunities to enhance current offerings within identified government agencies
