The DC Health Matters web portal was developed through a grant by the NIH National Center for Advancing Translational Sciences.
This work was further carried out by the DC Health Matters Collaborative (formerly the District of Columbia Healthy Communities Collaborative): a citywide collaborative of non-profit hospitals and community health centers. This cooperative effort established in 2012 conducts a community health needs assessment for Washington, DC every three years and develops the community health improvement process (CHIP) which is a community-based effort to address the health and well-being of the District of Columbia community.
